What is Hospice?
Hospice may be appropriate if you or your loved one has an illness that – if it runs its normal course – could take life in six months. People may be on hospice for weeks, months, sometimes years, because the course of many illnesses is impossible to predict. Lamm Hospice services provides compassionate medical care, eases communication and offers support to families for individuals not seeking aggressive treatment for their end-stage illness such as:

- Heart Disease/CHF
- HIV/AIDS
- Liver Disease
- Parkinson’s Disease
- Pulmonary Disease/COPD
- Renal Disease
- Stroke/Coma
Care Management
Levels of Care Provided:
Routine Home Care: Care is provided in the patient’s home, nursing home, or assisted living facility in the form of intermittent visits by our hospice team.
Respite Care: Continuous care is provided by our hospice team for up to five days when or if the primary caregiver is unavailable to provide care.
General Inpatient Care: Care is provided at an area hospital that cannot be administered in a home setting. Our hospice team will make daily visits and coordinate patient care with hospital staff and physician.
Continuous Care: Care is provided to patients who require continuous bedside care in the patient’s home by our hospice team for a limited time.
Lamm has partnered with nursing homes and assisted living facilities throughout our service area to provide care to their residents; as well as a number of area hospitals to manage the in-patient hospice units within those facilities.
Medical Equipment / Supplies and Medications
Hospice covers durable medical equipment (DME) and supplies related to your hospice diagnosis. This includes items like wheelchairs, hospital beds, diapers, oxygen and wound care supplies.
Hospice will obtain an order for your medications from the physician. Hospice provides medicines related to your primary diagnosis and will review the medication supply with you on visits to ensure you have medications needed. Hospice case managers will work with you to individualize interventions to effectively manage symptoms, medications, other treatments to decrease pain and help to improve your overall comfort.

Spiritual Care
Lamm Care offers spiritual guidance for patients and families of all denominations and multiple faiths. Our spiritual care counselors visit as often as the patient and/or family requests.
Bereavement Support
After a loved one has passed away, we know that grief is a process that takes time. Our professional bereavement counselors and volunteers stay in touch with hospice families and continue to provide compassionate support for 13 months to those who are transitioning through the loss of a loved one.
